Persons who receive their first Moderna vaccine from NET Health will receive notification of when to return to receive their second and final dose.
Please do not enroll yourself onto our waiting list multiple time so that you avoid receiving multiple invitations to make an appointment.
It is difficult to give an exact number or days or weeks until we will contact you to schedule an appointment.
After you complete our registration form and click the "Submit" button, the following page will be your Confirmation Page that you are on our waiting list.
We can help anyone to join our waiting list. Please call a Community Health Worker to be enrolled over the phone by calling (903) 617 - 6404.

Only one online registration per person is allowed so that we can ensure only persons on our waiting list are receiving the invitation to make an appointment.
When you receive the official invitation, you will be able to enter the other person's name within your online registration.
Disregard or delete any appointment invitations you may receive from NET Health so that we can give your spot to another person on our waiting list.
